Privacy notice
Public education and protected health workflows stay separate.
This public educational site enables one general, nonclinical inquiry form. It does not enable patient matching, provider applications, patient accounts, medical uploads, or advertising tools. Google Analytics 4 measures aggregate site use so the education library can be maintained and improved. LA Digital Systems, represented by Domenico Savatta, MD, FACS, is the privacy and retention owner.
What this site collects
The general inquiry form collects a name, email address, inquiry type, general question, and the required acknowledgment that the channel is nonclinical. It does not request phone numbers, birth dates, medical histories, insurance information, files, or payment information. Google Analytics 4 may collect limited device, browser, page-view, and interaction data, including a fixed event label after a successful form submission. Advertising personalization and Google signals are disabled. Standard infrastructure logs may be processed by Vercel, Cloudflare, and Opusite for delivery, reliability, security, and abuse prevention.
Ownership and routing
LA Digital Systems is the assigned privacy and retention owner. General nonclinical inquiries are stored in Opusite and a notification containing the submitted values is routed to the group address info@modernsleepandmetabolicinstitute.com. Corrections, access, and deletion requests can be sent to info@ladigitalsystems.com. If you use the email fallback, the message is handled by the sender and recipient email systems. Do not send patient information through either channel.
Retention and sharing
General inquiry submissions remain in Opusite until an authorized operator deletes them or the account is closed, subject to lawful retention requirements. Notification copies are handled by the recipient email systems. The form data is used to respond to and administer the general inquiry; it is not used to create a patient record, CRM contact, support ticket, or marketing subscription. Analytics and infrastructure providers may retain limited technical usage data under their service and security terms. Submitted data is not sold or used for advertising.
Before any intake opens
The general inquiry form is not an intake workflow. Any future ZIP-code, provider, patient, or protected-health workflow requires a separate approved notice covering the exact data, purpose, consent, access, retention, deletion, security, and applicable healthcare obligations before collection begins.
