Oximetry alone cannot diagnose or exclude obstructive sleep apnea because it does not record airflow, breathing effort, sleep time, arousals, or event type. Repeated oxygen drops can raise concern, but lung disease, altitude, artifact, or poor sensor contact can cause similar patterns. A clinician-selected home test or in-lab polysomnogram answers the diagnostic question.

What overnight oximetry measures

The sensor estimates arterial oxygen saturation from light absorption, usually at a fingertip.

A medical oximeter records oxygen and pulse through the night. Its report can show the baseline, lower readings, time under a threshold, and patterns of change. Interpretation also considers signal quality, altitude, circulation, heart and lung history, medicines, and sleep context.

What home testing and the laboratory add

More signals let a clinician classify events instead of inferring them.

A home test typically records airflow, effort, oxygen, and pulse. In-lab polysomnography also measures sleep stages, distinguishes obstructive from central events, and adds attended monitoring. AASM recommends the laboratory for significant cardiorespiratory disease, hypoventilation, possible respiratory muscle weakness from a neuromuscular condition, chronic opioid use, prior stroke, severe insomnia, or another suspected sleep disorder.

When each overnight test is typically ordered

The order reflects the question, not a ranking of test quality.

Oximetry assesses sleep oxygen, supports qualification, or monitors treatment. A home sleep apnea test is diagnostic for an appropriate candidate. The laboratory is used when a home result is negative or inadequate despite ongoing concern, or for titration or complex evaluation.

How an overnight oximetry report is read

The lowest number is not the whole result.

Reviewers consider baseline saturation, the size and frequency of dips, sustained low values, time below chosen thresholds, and valid signal time. Device definitions differ. The commonly cited mid-90s or higher range is mainly an awake reference, sleep can include some variation, and one minimum value is not a diagnosis or severity grade.

Why an oxygen pattern is not a diagnosis

The same trace can have several explanations, and some sleep apnea produces little desaturation.

Repeated dips can reflect breathing events, artifact, lung disease, altitude, or another process. Airflow limitation can also fragment sleep without dramatic oxygen change. Clinicians need signal quality, timing, symptoms, and diagnostic channels before assigning a label.

Cost and coverage questions before the test

Medical recordings and consumer gadgets sit in different billing lanes.

Medical oximetry may include device provision and physician review and may be billed differently from a sleep study. Medicare applies specific home-oxygen criteria; one low reading does not establish coverage. A consumer ring is usually a cash purchase. Confirm current criteria and included services with the ordering team and payer.

Questions after an abnormal or reassuring result

The next step depends on what remains unexplained.

Ask whether the recording was valid, whether oxygen was low continuously or intermittently, and whether heart or lung evaluation is relevant. If snoring, witnessed pauses, gasping, morning headaches, or daytime sleepiness continue after a reassuring trace, ask about diagnostic sleep testing.
