The medicine determines the baseline
Different mechanisms carry different contraindications and monitoring.
A clinician considers kidney clearance, liver disease, pancreatitis or gallbladder history, blood pressure and heart rate, seizures, glaucoma, opioid exposure, mood, absorption, and nutrient risk depending on the drug. One panel cannot replace a treatment-specific label and clinical review.
Metabolic tests establish current risk
Glucose and lipids help define related disease and treatment goals.
A1C or fasting glucose can identify prediabetes or diabetes; lipids inform cardiovascular risk; liver enzymes may support evaluation of steatotic liver disease; kidney function can affect selection or dosing. Test interpretation must account for fasting status, illness, pregnancy, and prior results.
Pregnancy planning is a hard safety gate
Weight-loss medicines are not used for weight loss during pregnancy.
Discuss pregnancy potential, contraception, fertility treatment, planned conception, and breastfeeding before prescribing. Some medicines require a defined discontinuation interval before pregnancy. The prescriber should provide exact instructions for the selected product rather than a generic rule.
Thyroid testing is targeted
Routine TSH may be reasonable in some evaluations, while extensive endocrine panels need indications.
Symptoms, examination, medicines, menstrual history, prior thyroid disease, and weight trajectory guide testing. A personal or family history relevant to a drug’s thyroid warning also belongs in the prescribing review. Normal thyroid results do not mean weight is a behavior problem.
Vitals and history can outweigh a lab
Blood pressure, pulse, sleep apnea, seizures, mood, and substance use can change safety.
Stimulant-like medicines require cardiovascular assessment; naltrexone affects opioid treatment; bupropion-related seizure and psychiatric considerations matter; sedating combinations can affect OSA. Bring PAP data and report dangerous sleepiness rather than assuming weight treatment will immediately resolve it.
Monitoring begins before treatment
A baseline is useful only when paired with a follow-up decision.
Write the starting symptom and risk measures, expected benefit window, dose-escalation plan, side-effect rules, repeat labs, and stopping criteria. Clarify how shortages, cost, missed doses, surgery, illness, or a pregnancy changes the plan. Avoid unlicensed compounded substitutes without a clinician-led risk review.
